£28,407 - £34,581 per annum/pro-rata

Job Description:

We are currently seeking a dedicated and passionate social worker to join our team. The ideal candidate will have a strong commitment to helping individuals and families in need, excellent communication skills, and the ability to wk collaboratively with other professionals.

Responsibilities:

- Conduct assessments of individuals and families to determine their needs

- Develop and implement care plans to support clients in achieving their goals

- Provide support and advocacy for clients within the community

- Work closely with other social services professionals to coordinate care

Requirements:

- A degree in social work or a related field

- Registration with the Health and Care Professions Council (HCPC)

- Previous experience working in a social work setting

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team
